Camp Forms

Forms are due by May 15th

  • Parent Authorization: Must Be Printed and uploaded to your Campers Account once completed. This form requires a signature from the parent/guardian & a copy of insurance card. 

  • Health History Form & Immunizations: (filled out & ELECTRONICALLY SIGNED by the parent)  This form must be completed and electronically signed by a parent or adult guardian of a camper under age 18. The information on this form is gathered to help us provide safe and appropriate health care. Please make sure immunizations are filled in and updated.

  • Behavior & Policies Agreement: (filled out by both parent and camper)  Parents and campers must complete this form before coming to camp. Use this time to have a conversation with your child before they come to camp. 

  • Camper Photo: Please upload a current photo. Many returning campers have very old photos on their profile.

Medication Form 
(only needed if bringing medication)

Please complete this form and bring to camp (Do NOT send in advance) along with properly labeled containers-must be in original containers/packaging.

We cannot accept expired medication.

Please complete the medication form accurately. Put all medication in a labeled clear Ziploc bag ready for a seamless drop off with our health care staff.

All medications must be turned in to the Camp Nurse during our Opening Day screening. This includes prescriptions, vitamins, and over-the-counter medications-even cough drops. These measures are in place to ensure that each camper receives the proper medication at the proper dosage. All unused medications can be picked up on Closing Day. Please do not discontinue medication while at camp to see how your camper responds at camp without it. In most cases, the results are disappointing.

PRINT MEDICATION FORM

Tentmate (Overnight Camp) or Groupmate Requests (Day Camp): This form can be completed or amended within 3 weeks of your camper’s session start date.
We want to ensure you have time to verify that your requests are registered for camp and that they are also requesting your child.

NO MORE THAN 2 REQUESTS. Requests must be mutual and contain first and last names spelled correctly. If you request more than 2 we will go by the first 2 names listed and disregard the rest. Campers must be within one year of age and be in the session and program.

We recommend discussing requests with the families of the requested before filling out this form. Please do not ask us to make acceptations-that is not fair to everyone else who follows the policy. Also please be aware if a request is not mutual and we are asked why we did not honor a request we will let them know it was not mutual.

We understand this can be a challenge for you but we are trying our best to make the camp experience the most rewarding and incredible experience for every child. Thank you for understanding!
